Lifeline and Link-Up Service Offerings

 As part of its service offerings, South Park Telephone offers two programs to assist qualifying low income individuals with the charges for their telephone service, Lifeline and Link-Up. These programs are part of the Federal Universal Service Fund program instituted by the Federal Communications Commission and overseen by the Universal Service Administrative Company.
 
Lifeline service offers a qualifying individual discounts to the costs of that individual’s telephone service for one telephone line per household at the principal place of residence. Link-Up allows qualified individuals to a discount on the cost of initially ordering telephone service. The current discount provided under the Company’s Lifeline service is $18.98 per month for each month that the customer qualifies. The one-time discount for Link-Up service is $29.00. To receive these discounts, an individual must be a participant in one of the following programs: Old Age Pension (OAP), Aide to the Blind (AB), Aid to the Needy Disabled (AND) or Supplemental Security Income (SSI).  
 
Other terms and conditions apply. For example, customers must apply for Lifeline and Link-Up, and we are required to retain and verify the continuing accuracy of the qualifying information provided by those individuals. Similarly, as long as a qualified individual agrees to toll blocking, that individual is not required to pay a deposit prior to our service being offered.  
 
We encourage you to contact us if you believe you may qualify for these programs. To confirm eligibility the subscriber should contact the Colorado Department of Human Services at: 1.800.782.0721 or South Park Telephone Company at 1.888.837.6400  
 
If eligible, the subscribers will receive written certification from the Department of Human Services and remit this confirmation to South Park Telephone Company.  South Park Telephone Company will apply the discount once it receives the certifications.
